- Sysadmin Service Request Form: This is the form to fill out if you need access to a club website/MySQL database or need a new user or club website/MySQL database created. NOTE: There has been a change in policy as of the 2009-2010 school year. All service request forms should be turned in to the eighth period office. They will then be processed by the Sysadmins.
- Sysadmin Application: This is the form to fill out if you would like to become a sysadmin. Please read this to learn more about becoming a sysadmin.
